Get the HLS media playlist for a range

Returns the HLS media playlist (video.m3u8) for the range: hourly windows are reused from cache or generated on demand, then stitched into one VOD playlist of presigned fMP4 segment URLs with an EXT-X-DISCONTINUITY between hours.
